A Clinical Review of 781 Cases of Biliary Tract Stones

Abstract
We report 781 cases of biliary tract stone disease who underwent an operation at the Department of Surgery, Wonju Christian Hospital, Yonsei University for 11 years from Jan. 1980 to Dec. 1990.
326 men and 455 women were in the series and the mean age were 53 years and 51 years respectively. The most common presenting symptom and physical finding were RUQ abdominal pain and tenderness. The accuracy of diagnostic tools were 94.6%(PTC),
91.5%(ERCP) and 89.4%(UP) respectively. Location of biliary stones was as follows; GB in 520 cases(66.6%), CBD in 369 cases(47.3%) and IHD in 137 cases(17.5%). Intrahepatic stones were found more frequently in left hepatic duct(89 cases). The
postoperative complication rate was 15.9% and the most common complication was wound infection(10.6%). The operative mortality was 0.9% and the most common cause of death was sepsis. The retained stones were found in 73 cases(9.4%) which were
located in
IHD(46 cases), in CBD(23 cases) and in both CBD and IHD(4 cases). To remove the retained stones, we performed choledochoscopy in 21 cases, reoperation in 19 cases, ESWL in 1 case and Monooactanoin in 7 cases. The rate of successful removal of
retained
stones was 16.6%. Choledochoscopic extraction of the retained stones was valuable before reoperation. The reoperation rate was 2.4% and successful result was obtained in 89.5% of the cases.
